Service Agreement


Welcome to Plan To Stay Occupational Therapy we look forward to working with you to achieve your client’s goals. This service agreement provides you with a summary of our fees and essential information that will assist us to provide the best possible care for your client.

Fees schedule

Play to Stay Occupational therapy services are provided at an hourly rate of $193. 

All new clients require an initial OT assessment. The initial assessment fee is $386, which includes the initial in person assessment, a written summary of recommendations, and up to 30 minutes travel time from Kingsford.

Further services will be charged at the hourly rate may include:

  • Follow up visits  (within 12 months)
  • High cost equipment prescription 
  • Detailed reports for home modification, equipment prescription and manual handling recommendations. (charged in 30 min increments)
  •  Client/carer education or training
  • Payment terms

Payment of invoices must be made within 14 days from the date of the invoice.

Payment of invoices to be made into the following bank account;

  • Plan to Stay Occupational Therapy
  • BSB: 063 097
  • Account number: 8209 0447
  • Client ID in description

Please state the client name in the reference of payment.

Cancellation policy

Plan to Stay Occupational Therapy has an attendance policy. Notice of cancellations must be received at least 24 hours prior to the scheduled appointment time or the FULL FEE of the session will be levied. The fee will be equivalent to the scheduled appointment fee. Notice of cancellation is accepted via phone call, text or email.

Confidentiality and Privacy

Plan to Stay Occupational Therapy protects client privacy and confidentiality information. Plan to Stay Occupational Therapy may be required to contact other health services, care providers or agencies involved in care of a client.

Termination of Service agreement

Should either party wish to end this Service Agreement they must give 1 week notice.
